If you are on America Online, you can send the completed form to the address CESoftware, and if you are on the Internet, you should send it to <Tom_Hillson%cedsm@uunet.uu.net> (the angle brackets are merely for decoration - don't include them in the address).

Tom will return a one-line reply within two working days, so you will know if you have gotten through (and don't send multiple messages to the different addresses).
