- QUESTION: When my hands began to shake, I thought it was just a matter of
- nerves, and gave up my smoking. Then, when it got worse, I began to think I
- had Parkinson Disease. Finally I visited my doctor who diagnosed this as
- "essential tremor". I would like to know how he could tell the difference and
- if there are any treatments that could help me now?

- ANSWER: While all tremors may look alike to a casual observer, a careful
- examination by a physician usually is enough to distinguish among the many
- possible diagnosis. The most common tremor disorder is "essential tremor",
- which is about 10 times more common than Parkinson's Disease. The trembling
- begins when the patient attempts to move, in contrast to the movement caused
- by Parkinson's which occurs when the patient is at rest. In Parkinson's the
- tremor is most common in the hands and legs, but never strikes at the head, or
- cause a quivering tone in the voice, which does occur in the essential tremor.
- A careful history may reveal a family history of a similar condition in
- essential tremor but is usually absent in Parkinson's. One of the interesting
- aspects of the history is to determine how small amounts of alcohol affect the
- tremor. Alcohol can dramatically reduce the tremors in the essential variety,
- but has no effect on the shakes caused by Parkinson's. Alcohol is also used
- as one of the treatments for the tremor, and may be very useful when a glass
- of wine or other alcoholic beverage is taken just before a meal. The calming
- effect makes eating quite a bit easier for those who hands are affected by the
- tremors. Essential tremor progresses quite slowly with long periods of time
- during which there seems to be no worsening of the symptoms. And it is
- doesn't shorten life expectancy, although severe tremors can make ordinary
- activities in life quite difficult to accomplish.
- Happily there are medications available that are quite effective. These
- are beta-blockers, including propranolol and primidone. Both are prescription
- medications, and require guidance from your physician to adjust the doses to
- your individual needs.
