- QUESTION: My daughter was positive on a test for tuberculosis at school, and
- so I took her to the doctor. He examined her thoroughly, and took other tests
- and an x-ray and told me that there was nothing to worry about. I heard there
- was a vaccination against TB, and asked that my daughter be given this, but he
- told me that was not possible. Why not? I would like to get all the
- protection for my daughter that is possible. What are my rights in this case?
- I need your help and guidance. Please answer.

- ANSWER: The positive test at school reveals that your daughter was exposed
- to tuberculosis at some time, not that she has the disease now. The skin test
- shows that the body produced antibodies to the bacteria, which could have been
- sufficient to overcome the infection in its initial stages. Certainly I would
- expect that had there been an ongoing problem, it would have been discovered
- by your doctor's careful examination and testing. In addition, your family
- should have been examined as well, looking for an active case of tuberculosis
- which furnished the exposure to the germ to your daughter. The use of BCG
- (Bacillus of Calmette and Guerin) is only indicated for children who live in a
- household with an active case of TB, at least according to the Advisory
- Committee for Immunization Practices at the Centers for Disease Control.
- There are several other bits of information you need to know. Though BCG
- has been widely used in some European countries, India and the United Kingdom,
- the results have been less than certain. The outcome in U.K. showed
- protection in only 77 percent of those immunized, while a 15 year trial in
- India showed no protective results from BCG immunization. However this may
- apply only to adults, and protection in children is generally higher. In
- addition there is only one form of the vaccine available in the United States
- at present, and it requires multiple punctures, not something everyone wants
- to endure. You have the right to seek another opinion, to request and receive
- more information about BCG immunization, and its potential risks and benefits.
- I suppose you have the right to demand the immunization, but the facts of your
- situation as you relate them to me would not, in my opinion, warrant the use
- of this procedure.
