Some places do have separate shipping for separate locations. CDnow does (though they may have changed that) so just check your shipping cart and it'll tell you. The worst instance was when I placed an order through CDUniverse (for DVDs) and the items were listed in stock but they shipped in three batchs and charged postage each time. I complained and got the shipping refunded since this wasn't something that had been agreed to.

CDNow sometimes will charge extra after the sale is supposedly
completed, which you have no way of rejecting. I complained
repeatedly but got no answer, and lodged a complaint with the BBB.
I haven't had a problem with Amazon doing this; when they've had to
ship from separate locations they've still charged me the agreed
price, and twice when an item with the "Ships within 24 hours"
designation was actually in the 2-3 day range, they bumped me to the
next better shipping method at no charge. But things could've
changed.

In my experience with Forced Exposure (I order 4 or 5 items from them
each week), they charge a flat $4.95 no matter how much you order. It's
a bit high for small orders but pays off on larger ones (and on not
worrying about how much shipping will be).
