  10.  AB    AIDS epidemic, deterioration of the public health infrastructure and
  11.        inadequate training of health care providers are some factors
  12.        contributing to the increase of tuberculosis (TB), especially
  13.        multidrug-resistant TB. In vitro drug susceptibility testing is now
  14.        required for all TB patients. Since faster turnaround times in the TB
  15.        laboratory are necessary, we compared the radiometric technique with the
  16.        conventional Middlebrook 7H10 agar method for susceptibility to
  17.        second-line drugs in a 3 phase study. In phase I, 10 susceptible M.
  18.        tuberculosis strains were tested in the BACTEC system against the
  19.        following drugs and concentration (mcg/ml): amakicin (AN, 1.0, 2.0,
  20.        4.0), capreomycin (CM, 1.25, 2.5, 5.0), ciprofloxacin (CIP, 0.5, 1.0,
  21.        2.0), clofazimine (CLO, 0.25, 0.5, 1.0), cycloserine (CS, 50, 60, 70),
  22.        ethionamide (ETA, 1.25, 2.5, 5.0), kanamycin (KM, 1.25, 2.5, 5.0),
  23.        ofloxacin (OFL, 0.5, 1.0, 2.0, 4.0). In phase II, specially selected 20
  24.        isolates with some strains resistant to each drug were tested. Drug
  25.        concentrations were adjusted based on the results in Phase I. CIP was
  26.        dropped and rifabutin (RBT, 0.5, 1.0, 2.0) was included. The following
  27.        preliminary critical concentrations (mcg/ml) for radiometric method were
  28.        established: AN 1.0, CM 2.5, CLO 0.25, CS 100.0, ETA 1.25, KM 2.5, OFL
  29.        2.0, and RBT 1.0. Final recommendations will be made after finishing
  30.        testing 30-50 recent clinical isolates in phase III.
