  10. Registering the Modem Doctor gives you access to one of the most powerful
  11. diagnostic tools you can have for your PC. Registration is simple.  Price
  12. for the Modem Doctor is $19.95 which includes US and Canada shipping
  13. charges. Payment can be made by Check or Money Order. We can handle COD's,
  14. however all shipping and handling charges for COD's must be paid by you
  15. and arraigned prior to shipment. Maryland residents should add the
  16. appropriate sales tax.

  18. International Orders

  20. International Orders should submit a money order payable in US funds. We
  21. cannot accept checks drawn on foreign banks that do not have a US bank
  22. affiliation. International orders should also include $2.00 for shipping
  23. and handling (except Canada where shipping is included).

  25. Remember...shipping is paid for in the US and Canada except for COD's.
  26. International Orders must include $2.00. All payments in US FUNDS, no
  27. checks drawn on a foreign bank that does not have a US affiliation can be
  28. accepted.

  45. If you are a registered user of an earlier version of the Modem Doctor,
  46. you can update to Version 4.0 for $5.00 + $2.00 for shipping.  All you
  47. need to do is include your Modem Doctor Serial Number (on your original
  48. disk or available when you first start the Modem Doctor) with this form.
  49. Please fill out the serial number information so we can send you the
  50. latest version of the Modem Doctor.
