  16. with a letter stating that the CD was on back order, but the package didn't

  18. have my membership card, the Dr's bio, or the autographed picture.

  20.      Has anyone else received their package and if so, are these items also

  22. missing from it?  I plan on writing to let them know, but would like to

  24. know if this is widespread or they just forgot mine.  Thanks.
