- I am having trouble getting the '\theindex' command to work
- correctly.

- I have headers set up which print chapters and sections,
- respectively, together with the page number. However, for
- the index headers and sections just stay unchanged from the
- previous chapter.

- I looked into the code of \theindex, and it does have a
- \@mkboth which I assume sets the headings. Nevertheless, all
- my index pages have the reader "references" (since that is
- the chapter that happens to be before the index).
