- the fulfillment house at 1-800-824-7779 (8:00 a.m.-5:00 p.m., Mountain Standard

- includes specific instructions on how to receive a cash refund and packaging
- materials to return the GC card. All return forms and packaging materials will
- be sent directly to the customer.
