Have you tried this from the cli:

    format drive <deviceName> name <name> FFS

(I'm away from my Amiga, but I beleive the syntax is correct)

You will be instructed to insert a disk, just hit return and it should
do an AmigaDos format.

In my case, the device name is dh0: and name is dh0.  I'm using the Spartan
interface and device driver.  There is a section in my mountlist for
dh0: which also calls out the fast file system and the spartan device
driver, and other things
