EMPLOYEE CODE OF CONDUCT
THE FOLLOWING CODE OF CONDUCT IS EFFECTIVE THIS DATE IN ORDER TO PROTECT THE
RIGHT AND SAFETY OF ALL EMPLOYEES AND RESIDENTS AT [ ]
[ ]
VIOLATION OF ANY OF THE FOLLOWING ACTS WILL CONSTITUTE CAUSE FOR DISCHARGE.
1.Stealing or willfully destroying or damaging any property of the facility,
its residents,visitors or personnel.
2.Disobedience or insubordination to supervisors.
3.Disorderly,immoral or indecent conduct.
4.Reporting for,or attempting to work while under the influence of,addicted to
alcohol,drugs or narcotics or in a physical condition making or
unsatisfactory to continue employment.
5.Punching time on another employee's timecard,or requesting another person
to punch your timecard.
6.Punching your timecard IN and OUT at times other than those authorized.
7.Discussing personal problems with residents.
8.Disclosing anything of a personal nature concerning a resident at anytime
either inside or outside the facility -unless the specific work duties
requires the giving or exchanging of such information.
9.Failure to exercise proper custodial responsibility of facility keys
assigned to your care.
10.Permitting another person to use keys to enter facility property
without proper authorization.
11.Unauthorized possession of firearms or other weapons on facility property.
12.Willful or careless disregard of or inattention to, working directions
and instructions;or refusal to comply with,or violations of these rules,
safety or fire regulations or sanitary rules and regulations.
13.Absence from work without a physicians permit and/or authorization from
supervisor.
14.Should you arrive to work early do not punch if prior to seven minutes
before your scheduled starting time and do not punch out more than
seven minutes after your scheduled ending time.
Permission to start work before or to continue work after the scheduled
time must be obtained in advance.If this permission is granted the
supervisor who granted it must initial the timecard.
15.Failure to obtain permission from supervisor to leave your job or
premises during working hours.
16.Smoking in unauthorized areas,resident rooms,corridors,treatment areas,
nursing stations,medicine rooms,kitchen,laundry.
17.Consuming food or beverages at unauthorized times or in unauthorized areas.
18.Selling tickets or chances on pools or raffles not promoted by the company.
19.Unauthorized posting of notices or literature on facility premises.
20.Soliciting,collecting funds,and/or circulating literature of any nature on
facility property during working hours without the approval of the
administrator/or Owner.

21.Performing personal work on facility time without the express permission
of the administrator/ or Owner.
22.Using facility business phone for personal matters, or making or receiving
personal telephone calls at any telephone other than the public
telephone and only then during authorized break and lunch periods.
23.Taking more than the specified time for meals or rest periods.
Failure to get authorization from supervisor or charge nurse to go on
breaks or lunch period. All employees must sign out or punch out on the
time clock.
24.Unauthorized attendance or participation in meeting, or gatherings during
actual working hours.
25.Discourteous conduct toward any resident, visitor,physician or employee.
26.Inefficiency,inability and/or gross or repeated negligence in the
performance of assigned duties.
27.Altering,falsifying or making a willful misstatement of facts on any
resident record or chart,job or work record,employment application
or any other hospital record,or chart.
28.Misrepresenting reasons when applying for a leave of absence or for other
time off from work.
29.Failure to return to work from an authorized leave or absence or vacation.
30.Failure to withdraw from,or report outside activities or interest
with conflict with,detract from,or adversely affect the interest of the
facility.
31.Leaving duty stations without notification to and authorization from
supervisor.
32.Being in unauthorized areas,kitchen,business office,pharmacy without
permission.
33.Having visitors while on duty without authorization.
34.Violation of Residents rights.
35.Arguing and/or fighting.
36.Threats of any nature.
37.Sleeping while on duty.
38.Watching television while on duty.
39.Being out of designated uniform and without name tag.
40.Failure to attend staff meetings without approval.
41.Leaving duty stations without proper relief or authorization.
42.Failure to obtain employee physical reports when requested by supervisor.
43.Lounging in residents rooms or using residents telephones.
44.Unauthorized use of company supplies,equipment or vehicles.
I understand all of the above and will abide by these rules of conduct while
on or off duty on the facility premises.
