Black triangles for new medicines

A black triangle should be put on the label of all dispensed medicines that are new enough for the Committee on Safety of Medicines to want all unexpected effects reported to them. Then patients can tell their doctors and the doctors can report the effects to the Committee. I want the Committee to propose this labelling officially to pharmacists, doctors and industry.
