  4. Members of the Hayes Sysop Program may use this form to trade-in
  5. their registered OPTIMA or ULTRA high speed products for Hayes OPTIMA
  6. 288 V.FC + Fax.
  7.  
  8. ATTENTION:  This offer valid only for registered members of the
  9.             Hayes Sysop program, and may only be used to upgrade
  10.             units purchased via the Hayes Sysop program.

  12. 1) You MUST be a registered member of the Hayes Sysop program in
  13.    order to qualify for this offer.  This means you must have
  14.    purchased a modem via the Hayes Sysop program.  If you are not
  15.    a registered member of the Hayes Sysop program and are interested
  16.    in trading-up to the Hayes OPTIMA 288 V.FC + Fax, please contact
  17.    Hayes Customer Support at 404-441-1617.

  19. 2) Only units purchased via the Hayes Sysop program are eligible
  20.    for this offer.  Only 9600 bps or 14400 bps ULTRA and OPTIMA
  21.    products are eligible.

  23. 3) You must return this form to Hayes either via fax, mail or BBS
  24.    and obtain a return authorization number BEFORE sending in your
  25.    modem(s).

  27. 4) Upon receipt and approval of the trade-in form, a Hayes
  28.    representative will call you with the return authorization and
  29.    shipping instructions.  Do not send the unit you are trading
  30.    until you have received your return authorization number.
