Members of the Hayes Sysop Program may use this form to trade-in
their registered OPTIMA or ULTRA high speed products for Hayes OPTIMA
288 V.FC + Fax.

ATTENTION:  This offer valid only for registered members of the
            Hayes Sysop program, and may only be used to upgrade
            units purchased via the Hayes Sysop program.

1) You MUST be a registered member of the Hayes Sysop program in
   order to qualify for this offer.  This means you must have
   purchased a modem via the Hayes Sysop program.  If you are not
   a registered member of the Hayes Sysop program and are interested
   in trading-up to the Hayes OPTIMA 288 V.FC + Fax, please contact
   Hayes Customer Support at 404-441-1617.

2) Only units purchased via the Hayes Sysop program are eligible
   for this offer.  Only 9600 bps or 14400 bps ULTRA and OPTIMA
   products are eligible.

3) You must return this form to Hayes either via fax, mail or BBS
   and obtain a return authorization number BEFORE sending in your
   modem(s).

4) Upon receipt and approval of the trade-in form, a Hayes
   representative will call you with the return authorization and
   shipping instructions.  Do not send the unit you are trading
   until you have received your return authorization number.

5) Incomplete forms will not be processed.

Return Authorization Method: You send in your Optima/Ultra(s) to Hayes
along with payment of $199US a unit and Hayes will send you Optima 288
V.FC(s).
 
Preship Method: Hayes will ship you Optima 288 V.FC(s) and charge $288US a
unit. Hayes will issue an RA Number for the return of the your Optima/Ultra(s).
Upon receipt of your Optima/Ultra(s), Hayes will credit you the difference.
Allow 2 to 4 weeks for credits to process.
