The anti-smoking hysteria hasn't arrived in Japan yet, and smoking is still very popular.
There have been anti-smoking-campaigns, and the Japanese know very well that smoking is harmful; nevertheless, the smoking rate among men is high, and the number of smoking women is increasing fastly. The Japanese government, believing in repressive laws on that issue, prohibits the smoking of cigarets to persons under the age of 20.
Cigarets can be bought in tabacco stores and at vending machines.

If you are a sensitive person concerning smoke, you may have a hard time in some situations in Japan. Non-smoking areas are not very common in restaurants and public areas, not even in family restaurants and fast food stores. Furthermore, you won't be able to enjoy pachinko, since the parlors are extremely smoky places.
On the other hand, all trains have non-smoking cars.
