Rowdy kids' parents should be liable

I would think that since the wild kids' parents obviously had their head
up their ass, and did nothing to bring them under control, they should
be liable for all the resulting medical expenses.

As for the safety concern of the playground equipment, you might see
about getting information on who in McD Corporate to contact. Hopefully,
they will take quick action, and install a softer matt, and do whatever
else is needed. Of course, no design is going to be absolutely risk-free.
The other parents who sit on their butt need to get with it and teach
their young-uns civilized behaviour.
