  9.  AB    OBJECTIVE: The relationship between plasma-levels of IL-2R and IL-6,
  10.        known prognostic markers of HIV-infection (CD4 count, p24 antigenemia)
  11.        and progression into symptomatic disease in children was investigated.
  12.        METHODS: 10 children (5 male, 5 female, aged 3-7 years) with perinatal
  13.        HIV-infection in different clinical stages were included in a
  14.        prospective study with follow-up over a one-year period. At study entry
  15.        2 pts. were classified in stage P1B, 3 in P2A, 2 in P2AF and 3 in AIDS.
  16.        Four children were p24-antigenemia negative and had CD4 count > 400/mm3
  17.        (> 20%) (Group I), while 6 pts. had positive p24 antigenemia and CD4
  18.        count < 400/mm3 (< 20%) (Group II). Every 3 months clinical/nutritional
  19.        status, CD4 count and p24 antigenemia were evaluated and plasma samples
  20.        were stored for determination of IL-2R and IL-6 levels by ELISA-testing.
  21.        RESULTS: Levels of IL-R ranged from 217-884 U/ml. There was significant
  22.        difference between IL-2R levels found in Group I (mean value 248 U/ml,
  23.        range 217-289 U/ml) and those found in Group II (mean value 360 U/ml,
  24.        range 235-884 U/ml) (p < 0.001). One child of Group II had disease
  25.        progression in AIDS during follow-up (CMV-chorioretinitis) preceded 4
  26.        months before diagnosis by 2-fold increase of IL-2R levels.
  27.        Plasma-levels of IL-6 ranged from 0-270 pg/ml. Positive determinations >
  28.        10 pg/ml were found significantly more frequently in patients in stage
  29.        P2AF and AIDS (p < 0.02) and exclusively in children with deficit of
  30.        weight for height (wasting score < 97) (p < 0.001). No clear
  31.        relationship between CD4-count, p24 antigenemia and plasma IL-6 levels
  32.        was found. CONCLUSION: Monitoring plasma-levels of IL-2R and IL-6 may
  33.        have prognostic value in perinatal HIV-infection. Further observations
  34.        would be needed to confirm these findings.
