An obese 58-year-old black male is seen in the office with complaints of
moderate dull aching pain of the low back. Pain is noticed more on the
right side and extends into the buttocks and hip. The pain has been
experienced intermittently over a period of six years, but has become
persistently worse over the past two weeks.
The patient is an account executive for a major financial firm who was
recently promoted to the position of Director of Accounts for a large
industrial account. His past medical history is unremarkable, with the
exception of two elevated blood pressure readings taken in the past three
months. The patient has not been hospitalized previously and has no known
allergies. Family history is positive for osteoarthritis and
cerebrovascular accident.
