  1. The diagnoses lists and procedure lists included on this disk
  2. are not perfect nor comprehensive. If you are willing to perfect
  3. certain code areas, please do so and submit changes to AAMSI
  4. Software exchange. Basically we are interested in the clinical
  5. terms and synonyms that are used in patient care by the provider
  6. who fills out a route sheet, encounter sheet, superbill, medical
  7. record face sheet, or clinical research data sheet. We are not
  8. interested in the official ICD-9 or CPT-4 terminology except
  9. when it concurs with clinical use. The intended purpose of these
  10. lists is for database dictionary lookup for medical software
  11. of any type. That is why there are sometimes multiple terms for
  12. specific codes. Although there are probably instances of clinical
  13. terms for which no suitable code exists, it is best to try to
  14. force the term into a less specific code.
