  3. FIELD No.        What You Fill In

  5. 1 thru 6   Application Name as it will appear on the Menu.
  6. 7          Name of Secondary (Right Side) menu if any
  7. 8 thru 14  Application Names for Secondary Menu. 
  8. 15         DOS Command description for DOS Menu Selector 5.
  9. 16         DOS Command description for DOS Menu Selector 6 (Replaces
  10.            the Secondary Menu Name display)
  11. 17         Chirp ON or OFF & 12 or 24 hr.clock display (default is ON & 24)
  12. 18         Default Directory Name to Return to when application is completed.
  13. 19         Default Pathnames (overides any set in AUTOEXEC.BAT)
  14. 20         Menu Name as it appears in the Title Bar.
  15. 21 thru 34 DOS Pathname and Application Name (relating to 1-13 on the left)
  16.            Directory Name first, must be preceeded by '\' just as if you
  17.            were to issue a 'CD \Directory' in DOS.  This is followed by a
  18.            '\Application name' as if you were typing '123' to enter Lotus.

  22. 36         DOS Command that corresponds with item 16.
