  13.         report which was used for my credit evaluation.

  15.                 The Fair Credit Reporting Act of 1970, Section 609
  16.         provides the credit bureau should send me information which led
  17.         to denying my credit application. According to the provisions of
  18.         Section 612, there should be no charge for this information.

  20.                 Please send my credit report to me at the below address.
  21.         Additional information identifying my account is on the attached
  22.         letter that denies me credit.
