  18. A custom form created by a user may not be listed in the list of available
  19. forms when the user attempts to assign the form to a source.

  21. CAUSE

  24. One possible cause is that if the newly created form has margins defined
  25. greater than what the printer driver has defined, the newly created form
  26. will not show up. This is true even if the printer is capable of handling
  27. the margins you specify.

  32. Microsoft has confirmed this to be a problem in Windows NT and Windows NT
  33. Advanced Server version 3.1. We are researching this problem and will post
  34. new information here in the Microsoft Knowledge Base as it becomes
  35. available.

  40. Steps to Reproduce Problem
  41. --------------------------

  43. 1. In Print Manager, create a printer.
  44.  
  45. 2. Select the Forms menu option.
  46.  
  47. 3. Define a custom form using a paper size that the printer is physically
  48.    capable of printing, but is larger than 8.5-by-11 inches.
  49.  
  50. 4. From the Printer menu, choose Properties and then choose Setup. Attempt
  51.    to assign the newly created form to a source.
  52.  
  53. At this point, the form may not show up in the list of available forms.
