  19. of sound mind, willfully and voluntarily make known my desires
  20. that my moment of death shall not be artificially postponed.

  23.     If at any time I should have an incurable and
  24. irreversible injury, disease, or illness judged to be a terminal
  25. condition by my attending physicians who has personally examined 
  26. me, and has determined that my death is imminent except for death
  27. delaying procedures, I direct that such  procedures which would
  28. serve only to prolong the dying process be withheld or withdrawn,
  29. and that I be permitted to die naturally with only the
  30. administration of medication, sustenance, or the performance of
  31. any medical procedure deemed necessary to provide me with comfort
  32. care.
