The Homeowners Association Proxy is a document used by a member of a homeowners association to permit another person to represent and vote for the member at a homeowners association meeting.

Enter the name of the Homeowners' Association. This may be an unincorporated group or a non-profit corporation. The name of the association is generally provided in the Bylaws or other organizational document of the association.
Enter an X if the Owner should be referred to as "Homeowner" throughout the Proxy document. The association documents and rules (commonly called "Bylaws") will indicate the name by which to refer to the Owner. The hints will continue to refer to the Owner as "Homeowner."
