  4. The former 'Author-Direct' distribution service (for ASP members) and the 
  5. former 'ESC FDN' distribution service (for ESC members) have now been merged 
  6. into a single distribution service! This new service is called the "Author 
  7. Direct Distribution Service' (or simply 'ADDS'). This new service, along with 
  8. this updated submission form, will allow authors of both organizations to now 
  9. use a single submission form for the overall distribution of their products.

  11. Do NOT alter the format of this document.  This form should be completed using 
  12. a text editor or word processor (saved as a standard ASCII file). A separate 
  13. form is required for each file submitted. Limit your responses to the spaces 
  14. indicated, please. This submission form must be sent in hard copy form (due to
  15. signatures required), along with your file and processing fee, to the address 
  16. listed. See ADDSxx.TXT for additional information and address.
