  20. Perish, Inc.  Send a brief note with your address to
  21. spivak@math.rice.edu and ask for an order form to be sent to you.
  22. I've ordered books from Publish or Perish before, and have always been
  23. satisfied -- especially when a defective copy was promptly replaced.
