  22. I would have thought the address was in your wife's packet, but for those 
  23. that need it, all you have to do is write to Airline Antitrust Litigation
  24. PO Box 267, Pennsauken, NJ 08110-0267 and request the form.

  26. Note that it only applies to travel through certain hub airports and that
  27. you may have to provide proof of travel if you claim more than a certain
  28. number of trips.
