- | o All attendees must be DECUS U.S. Chapter members. If you are not, please |

- | charge for membership. |

- | o Registrations must be postmarked by August 14, 1992 to receive the pre- |
- | registration fee and must be accompanied by full payment. After this date, |
- | do not mail registrations; on-site registrations will be processed at the |
- | conference. |

- | o The on-site registration fee will be $65.00 for the conference, and |
- | $410.00 for the seminars. You may register on-site for seminars on Thursday |
- | only. |

- | o No purchase orders or government training forms will be accepted. |

- | o One person only per form, please. Duplicate this form if necessary. |
- | o Conference Cancellations: A $30 cancellation fee will be charged prior to |
