You may fill it in anonymously. Your anwers will ONLY be used for
building a statistical overview of computer virus incidents and the
damages caused by xomputer viruses.

- When (what date) was the virus discovered?
- Where (city, country) was the virus discovered?
- What virus was it?
- How did you discover the virus (by accident, virus synptoms, with a
  virus scanner or other tool)?
- Do you have a copy of the virus available for analysis?
- How many infected, machines, diskettes or harddisk were there?
- Has the virus caused you any damage like loss of productive hours,
  lost or damaged data? What was the amount of direct and indirect
  damage (in US dollar)?
- Have you been able to remove the virus from the infected
  machines(s)? IF so, by what method?
- What were the cost of this recovery (in US dollar)?
- Does your insurance cover damages caused by computer viruses?
- Have you reported the incident to the authorities (e.g. police)?
- Do you know how the virus got to infect your machine(s)? Do you know
  where the virus came from?
- Did you have any knowledge about computer viruses and were you
  taking any preventive actions against viruses? If so, what type of
  prevention was used?
- Will you take preventive actions against viruses in the future? If
  so, what type of prevention?
