Although humans did not make official contact with extraterrestrial intelligence until the 21st century, the planet received several visitors from space before then. Among these was a small Ferengi vessel that landed near Roswell, New Mexico, in July 1947. (“Little Green Men” [DS9]). In the past, social inequities plagued Earth. Symptomatic were 21st-century relocation camps called Sanctuary Districts, established in American cities, isolating the homeless, the unemployed, and the mentally ill from the rest of society. The Sanctuary Districts bred one of the most violent civil uprisings in American history, known as the Bell Riots, in September of 2024. This bloody revolt changed American public opinion about the Sanctuaries and forced the people of the United States to finally begin dealing with their social problems. (“Past Tense, Part I” [DS9]). During World War III, millions of people perished in a massive nuclear exchange that destroyed most of the major cities on Earth and eliminated most national governments. A dark decade ensued, during which most of the planet reverted to barbarism. This postatomic horror began to ease on April 4, 2063, when space pioneer Zefram Cochrane conducted his planet’s first faster-than-light spaceflight. Cochrane’s flight was directly responsible for Earth’s first contact with an extraterrestrial civilization, the Vulcans, on the following day. The event gave humanity the perspective it needed to begin a remarkable renaissance, bringing a virtual end to war, poverty, and disease over the next five decades. (Star Trek: First Contact).Earth’s first world government was established only a few decades later, in 2150. (“Attached” [TNG]). Addendum: Third planet in the Sol system. Millions of years ago, Earth was dominated by a wide variety of reptilian life-forms, including those later known as dinosaurs. Most dinosaurs became extinct at the end of the Cretaceous Period some 60 million years ago. However, some hadrosaurs survived on an isolated land mass, where they evolved into an intelligent species that eventually became known as the Voth. Although the Voth homeland on Earth was eventually destroyed, the technologically advanced Voth had by that time developed spaceflight capability, and survived by traveling into space. Addendum: Starfleet officer who joined the Maquis terrorist group. Eddington married fellow Maquis member Rebecca Sullivan in 2373. Two weeks later, he was arrested by Starfleet authorities. He was released several months later when it was feared that Eddington’s Maquis group had engineered a devastating missile attack against Cardassia Prime, an attack that could have drawn both the Cardassians and the Federation into an all-out war. Captain Benjamin Sisko transported Eddington to the Badlands to persuade him to provide information on the suspected sneak attack, but learned that there was in fact no attack in progress. The apparent attack was a fake engineered by Eddington’s wife. Once in the Badlands, Eddington was killed by Jem’Hadar troops while helping Sisko evacuate the surviving members of Eddington’s Maquis group from planet Athos IV. Rebecca Sullivan was among the survivors. (“Blaze of Glory” [DS9]).

When Emck learned of this, he elected to continue dumping anyway, until he was forced to discontinue the practice by the Federation starship Voyager. (“Night” [VGR]).

The EMH backup module from the Federation starship Voyager was stolen by followers of Tedran in the Delta Quadrant in late 2374. The module was subsequently lost for several centuries. Kyrian historian Quarren discovered the unit in 3074 under ruins in Kesef. Once reactivated, the backup holographic doctor served as an eyewitness to a critical period in ancient Kyrian history. Quarren and other Kyrian scholars were startled to learn that Kyrian assumptions about the 24th century were seriously mistaken. Voyager's backup EMH later served the Kyrian and Vaskan people as Surgical Chancellor, a post he held for many years. Eventually, curious as to the fate of his friends on the Starship Voyager, whom he missed very much, the backup EMH took a small craft and set course for the Alpha Quadrant. (“Living Witness” [VGR]).

The Emergency Medical Hologram was developed at Starfleet’s Jupiter Station and was designed by Dr. Lewis Zimmerman. Lieutenant Reginald Barclay was a member of Zimmerman’s development team, in charge of testing the EMH’s interpersonal skills. (“Projections” [VGR]). The EMH program caused holographic projectors not only to generate an image of a person, but also to create a magnetic containment field, within which electromagnetic energy was trapped, thereby giving the Doctor the ability to physically manipulate real objects such as patients and medical instruments. (“Phage” [VGR]). The Sovereign -class Starship Enterprise -E, launched in 2372, was also equipped with an Emergency Medical Hologram. Enterprise -E chief medical officer Beverly Crusher was not fond of the EMH program, although he proved useful in protecting the ship’s medical staff during the Borg invasion of 2373. (Star Trek: First Contact). Robert Picardo made a cameo appearance as the holographic doctor in Star Trek: First Contact. Zimmerman’s team also worked to develop a more sophisticated version, called the long-term medical hologram (LMH). (“Doctor Bashir, I Presume?” [DS9]).
After the entire medical staff of the U.S.S. Voyager were killed in 2371 during the ship’s rough passage to the Delta Quadrant, the ship’s Emergency Medical Hologram became the only source of medical treatment for the crew. (“Caretaker” [VGR]).
The Doctor was programmed with over five million possible treatments, with contingency options and adaptive programs (“Ex Post Facto” [VGR]) utilizing sophisticated multitronic pathway programming. (“The Swarm” [VGR]). He was programmed with information from 2,000 medical references and the experience of 47 physicians. (“Parallax” [VGR]). The EMH program, which was first activated on stardate 48308, consisted of more than fifty million gigaquads of computer data and was equipped with the medical knowledge of more than three thousand cultures. (“Lifesigns” [VGR]). His knowledge of medical treatments included those based on psychospiritual beliefs such as those employed by some of Earth’s Native Americans. (“Cathexis” [VGR]). The EMH’s programming was extremely sophisticated, permitting him to learn from new data and experiences, and even to be creative. Shortly after stardate 48532, when crew member Neelix’s lungs were removed, the doctor saved Neelix’s life by devising a means of creating a pair of holographic lungs, using the holographic emitters in sickbay. (“Phage” [VGR]). The EMH was definitely capable of independent thought. In 2372, he refused a direct order to separate Tuvix into Tuvok and Neelix. He did so because obeying the order would have required him to take a life, violating his doctor’s oath to do no harm. (“Tuvix” [VGR]).
So sophisticated was the holographic doctor’s program that he was a sentient life-form. He therefore found it frustrating when some members of the Voyager crew treated him as an inanimate computer program. Captain Janeway ordered that the EMH be given control over his own deactivation sequence, in order to avoid the indignity of his being deactivated by others. (“Eye of the Needle” [VGR]). The Doctor’s role as sole medical professional on the ship caused him to undergo significant growth as a sentient life-form. One of his early steps in this growth process was his search for a name for himself. Among the names he considered were Benjamin Spock and Jonas Salk. (“Ex Post Facto” [VGR]). He also thought about Albert Schweitzer. (“Heroes and Demons” [VGR]). Vidiian physician Danara Pel suggested Shmullus, after her beloved uncle. (“Lifesigns” [VGR]). (In one possible future visited by Kes, the holographic doctor aboard the Voyager decided on the name Dr. van Gogh, after being known as Dr. Mozart for a time.) (“Before and After” [VGR].) The doctor was not programmed to bleed or to feel pain. (“Projections” [VGR]). When activated, the Emergency Medical Hologram established communication links with all key areas of the Starship Voyager. He sometimes used this ability to surreptitiously listen to conversations throughout the ship, until Captain Janeway ordered him to refrain from eavesdropping. (“Parturition” [VGR]). The Doctor was sensitive to criticisms that, as a synthetic life-form, he would have difficulties empathizing with the suffering of an organic patient. In order to address this potential weakness, he once programmed himself with a holographic simulation of the 29-hour Levodian flu so that he could experience the disease process. (“Tattoo” [VGR]). His programming did not include the ability to cry. (“Threshold” [VGR]).
The EMH undertook his first away mission on stardate 48693. He was dispatched to the ship’s holodeck to investigate the disappearance of three Voyager crew members. He was successful in interacting with a photonic being that had taken up residence on the holodeck and in gaining the return of the missing crew. His actions during this first contact mission gained him a special commendation from Voyager Captain Janeway. (“Heroes and Demons” [VGR]). On stardate 48892, a surge of kinoplastic radiation caused a malfunction in Voyager’s holodeck circuits, trapping the Doctor in a delusional reality in which he briefly believed he was a real person named Lewis Zimmerman. In this fantasy, the Doctor imagined he was married to a human woman named Kes Zimmerman. (“Projections” [VGR]). In 2372, he fell in love with Vidiian Danara Pel. (“Lifesigns” [VGR]).
The holographic doctor was designed as a short-term supplement for medical personnel, not to be used for more than 1,500 hours. By stardate 50252.3, this limit had been greatly exceeded, and the EMH suffered level-4 memory fragmentation, resulting in rapid deterioration of program function. Rather than re-initialize the EMH, which would caused him to forget everything he had learned, including growth of personality, the Voyager crew instead decided to merge the EMH’s program matrix with that of the Jupiter Station EMH diagnostic program. The matrix overlay process worked to maintain the integrity of the EMH, but his memory was not fully restored. (“The Swarm” [VGR]). When the Voyager visited Earth’s past of 1996, Henry Starling fitted the EMH with an autonomous holo-emitter. When the Voyager’s crew returned to their own time period, the doctor retained the holo-emitter, which gave him the ability to operate outside of sickbay, even in areas without holographic emitters. (“Future’s End, Part II” [VGR]).
In 2373, the Emergency Medical Hologram created a personality-improvement holographic program that incorporated several historic characters’ personalities into his subroutines. Unfortunately, the unsavory side of these characters turned the EMH to evil until B’Elanna Torres purged the personalities from his memory. (“Darkling” [VGR]).
The holographic doctor’s first appearance was in “Caretaker” (VGR). During the early days of Star Trek: Voyager, the show’s producers had planned for the Doctor to choose the name Zimmerman for himself, and scripts for the show’s first season referred to the character as Dr. Zimmerman. By the end of the first season, this plan was changed, and later scripts referred to the character simply as the Doctor, although the scientist who developed the program became known as Lewis Zimmerman, who was also played by Robert Picardo. The holographic doctor’s eventual name, if any, remains a mystery at this writing.
Addendum: Chief medical officer aboard the Starship Voyager. Voyager's EMH was activated on stardate 48315. (“Year of Hell, Part I” [VGR]). The Doctor strove continuously to improve himself and his ability to serve the Voyager crew. In one such effort in 2373, the Doctor devised a custom holodeck program to permit him to experience human family life. He found the program to be surprisingly profound, as he had not anticipated the depth of emotions triggered at the death of a family member. SEE: Belle; Doctor’s family program Beta-Rho. (“Real Life” [VGR]). On stardate 50912, the EMH’s optical sensors were reconfigured so he could see into the microwave range of the electromagnetic spectrum. (“Displaced” [VGR]). In 2374, the EMH traveled to the Alpha Quadrant and was successful in informing Starfleet Command of the fact that the Voyager had not been destroyed three years previously, but had in fact been pulled across the galaxy to the Delta Quadrant. Emergency Medical Hologram programs are not equipped with genitalia; however, the Doctor altered his program to allow for sexual relations. (“Message in a Bottle” [VGR]). It is somewhat unclear with whom he had sex, but speculation might point to Danara Pel (“Lifesigns” [VGR]). The Doctor learned, however, that not all improvements were positive experiences. Around stardate 51658, using a new psychiatric subroutine, the Doctor recovered memories from Seven of Nine’s mind that had apparently been repressed after a traumatic experience. This evidence, which appeared to implicate an Entharan trader of a serious crime, was later found to be erroneous, but resulted in the death of the trader. The Doctor later felt great remorse for his errors in the case, and Voyager captain Janeway expressed confidence that the experience would help him perform his duties with greater wisdom. A team from Deep Space 9 conducted a salvage mission to Empok Nor in 2373 to obtain abandoned Cardassian hardware for use on the former Terok Nor. As a precaution against such unauthorized intruders, three soldiers of the Cardassian First Order’s Third Battalion were left behind in stasis to defend the station if need be. Their stasis chambers were programmed to revive them if station sensors detected an intruder. The soldiers were treated with powerful psychotropic drugs, giving them a heightened sense of paranoia toward outsiders, making them even more effective at defending the station. (“Empok Nor” [DS9]). SEE: pattern scrambler. Nearly a year later, a team of mercenaries led by Quark met at Empok Nor with a Dominion contingent for the purposes of a prisoner exchange. (“The Magnificent Ferengi” [DS9]). Empok Nor and its interior sets were, of course, re-dresses of standing sets of station Deep Space 9.

Commander William Riker was ordered to pilot a shuttle to the Endeavour to spread the Ktarian takeover. (“The Game” [TNG]). The Endeavor was part of the Starfleet armada that intercepted a Borg cube at Earth on stardate 50893. (Star Trek: First Contact). The Starship Endeavour was named in honor of British explorer James Cook’s flagship and for NASA’s space shuttle.
Addendum: Federation starship. The Endeavour was commanded by Captain Amasov, and was the only starship to survive the disastrous encounter with the Borg at Wolf 359 in early 2367. (“Scorpion, Part I” [VGR]). The Endeavour appears to have been the sole survivor of the battle of Wolf 359, although this was not explicitly established in any episode. We know that the Endeavour was still around in 2373 because it was part of the armada that met the Borg in Star Trek: First Contact. Nevertheless, Amasov’s first-hand experiences with the Borg (from which Janeway read in “Scorpion, Part I”) had to have been recorded before the Voyager departed from Deep Space 9 in 2371, which was before the Borg battle in Star Trek: First Contact. It therefore seems likely that the Endeavour (and Amasov) were also at Wolf 359 in 2367.
