I am baffled!

I have used Crossdos, multidos, and messydos, and have tried to format
an msdos diskette under all of the above, as well as under Amigados
1.2, 1.3, and 2.04 operating systems, and even on two different A500's
with all of the above, but after the disk is formatted and verified
sector-by-sector, initialization is attempted, at which time I get
a "format error" message.

What's going on?
