  24. You could easily make your own.  I sort of made one before (moreso as a joke
  25. than anything else...unfortunately now-a-days, it seems almost NECESSARY).

  29. event that is even partially related to sex, including any form of
  30. intercourse and/or penetration with _____________________, as of this date
  31. forward, _________________.  Also, in doing so, I agree not to press any
  32. charges of rape, sexual assult, assault, or any other such related
  33. sexual or non-sexual crime with the above mentioned person.

  39. going to make one, make it right...don't leave any chance for error that
  40. could be used against you in court (i.e. "Well, he didn't state that if
