  10. From what I know/remember, the criteria are:
  11. 1)If a extra player leaves the bench to stop a breakaway.
  12. 2)  In the last 2 minute of regulation or any time in OT: a player
  13. intentionally shoots the puck out of the rink, freezes play
  14. intentionally, removes a net from its moorings as to stop a goal, or
  15. goaltender removes helmet/facemask.
  16. 3) The one about the breakaway: player in possession of the puck
  17. (control!)  in his attacking zone with no one except the goaltender to
  18. pass, is interfered with from behind = penalty shot.  (note- the player
  19. is not awarded a penalty shot if he/she gets a shot on net.)
  20. 4)  Any defending player except the goaltender covers the puck in the
  21. goal crease.
  22. 5)  If the goaltender 'throws' his/her stick to prevent a goal at any
  23. time.  (it's a judgement call to whether the tender throws his stick,
  24. usually)
