All you need is the three connectors (S-VHS and 2 RCAs) and some length of 75 ohm coaxial cable. There is no need for any impedance matching (other than the requisite terminations at the monitor, probably built in) or level adjust. Two of the 4 mini-DIN (that's the S-VHS connector) are grounds; they are the two pins most distant from each other and closest to the non-conductive guide pin.
