  5. 19366 USA.  Payments must be in US dollars drawn on a US bank, or
  6. you can send international postal money orders in US dollars.

  8. CREDIT CARD ORDERING:  For information on ordering by MasterCard,
  9. Visa, American Express, or Discover, using phone, FAX, e-mail, or
  10. postal mail, see the section titled Credit Card Ordering on the
  11. page following the order form.

  17. SITE LICENSES: A site license for FileKick entitles an organization
  18. to receive one copy of the distribution package and duplicate the
  19. distribution disk for the specified number of copies.

  21. PURCHASE ORDERS: (net 30 days) are accepted only from government
  22. and accredited educational institutions and major corporations,
  23. provided that they are submitted on purchase order forms with a
  24. purchase order number.  Please be sure to include the standard
  25. FileKick order form with a purchase order.
