  5.         Fill page 2 and 3 of this form out and send it with proof of

  14.         When entering your "registration name" below, remember that
  15.         this is the actual text shown on screen by the registered
  16.         version, so you may use either your own name, the name of
  17.         your BBS/Mailer or a company name.

  19.         You can recieve your personal registered version by mail or
  20.         via file attached email. If you choose file attach, remember
  21.         to fill out the applicable parts of page 2 for the delivery.
  22.         Note that this option is only valid for listed CM-capable
  23.         nodes of fidonet and virnet.

  28.         Regardless of method, the license fee should be made payable
  29.         to NewAge productions and in swedish currency SEK.
  30.  
  31.         You may use any of the below payment methods:

  33.         ■ By Postal Giro. The Postal Giro account is 41 38 03 - 8.
  34.           Postal Giro is also known under names like PostGiro, Compte
  35.           Chèques postaux, Postscheckkonto, PostRekening or Girobank
  36.           Account depending on country origin.

  38.         ■ Sending money directly by mail.
  39.  
  40.         ■ By postal money order or international money order. Note that
  41.           an extra charge of 40 SEK is added due to the bank charges.

  43.         ■ By credit card. Note that an extra charge of 40 SEK is added
  44.           due to the bank charges. IMPORTANT: If you pay by credit card,
  45.           you have to send this form as normal mail, not by fax or
  46.           netmail. Credit card orders are handled by FenrisUlven Data AB,
  47.           so don't get confused when you see that on your credit card
  48.           account reciept. Don't forget the signature!
