  4. INSTRUCTIONS FOR FILLING OUT THIS FORM:  I sincerely want to resolve any
  5. problems you encounter with my doors but to do this I must have accurate
  6. and concise information on the problem. Too many people call me and expect
  7. me to resolve their problem on nothing more then "it does not work". That
  8. simply will NOT work to resolve the problem.  Before you fill this form out
  9. I request that you complete the B4UASK.DOC checklist to make sure you have
  10. not forgotten to do something in the setup of the game.  If that does not
  11. alleviate the problem then the next step is to completely fill this form out
  12. AND package it up with any error logs, batch routines and the CTL file
  13. and either freq it to me at my FIDO address, upload it to my BBS, or email
  14. it to me at mburkepei@hotmail.com
  15. Please note that I usually do not respond to inquiries unless it is either
  16. through one of the above listed communication methods.
  17. If you want a reply within 24-48 hours then you must either log onto my BBS
  18. OR you must poll back for my reply to your message.  I do not check e-mail
  19. very frequently.
