  1. 4400.BLD       Simple Power of Attorney
  2. ~This is a power of attorney that provides for the major
  3. ~reasons why a power of attorney may be desired including 
  4. ~obtaining records, executing documents or acting for another.
  5. 8415.BLD       Durable Power of Attorney
  6. ~This form is used for creation of a power of attorney that does
  7. ~not lapse upon the incapacity of the principal (person giving
  8. ~the power).
  9. 15000.BLD       Medical Power of attorney for health care
  10. ~This form is used in conjunction with living wills to provide
  11. ~a mechanism for a person's desire as to medical care being made
  12. ~known in the event of that person's disability.

  41. ~Due to disability a "conservator" (person to manage assets) may
  42. ~be appointed. Most state guardianship/elder laws provide that a
  43. ~person may nominate a conservator in case one is needed in the
  44. ~future.
