" WORK RULES "
IN ANY FACILITY WHERE PEOPLE WORK TOGETHER, THERE ARE CERTAIN RULES OF CONDUCT
WHICH MUST BE IN EFFECT TO OPERATE IN AN ORDERLY AND EFFICIENT MANNER.
THE PURPOSE OF OUR WORK RULES IS NOT TO RESTRICT THE RIGHTS OF ANYONE, BUT TO
PROTECT AND DEFINE THE RIGHTS OF ALL EMPLOYEES. WE ARE SURE THAT OBSERVANCE
OF THESE RULES WILL LEAD TO A BETTER UNDERSTANDING AND RELATIONSHIP AMONG
ALL EMPLOYEES.
IT IS IMPORTANT THAT EVERYONE TREAT OTHERS WITH CONSIDERATION AND RESPECT.
WE WANT THIS TO BE A GOOD PLACE TO WORK. THEREFORE, SO THAT THERE MAY BE NO
MISUNDERSTANDING ABOUT SATISFACTORY CONDUCT, WE HAVE ADOPTED THE FOLLOWING
RULES AND DISCIPLINARY ACTIONS FOR VIOLATIONS OF WORK RULES.
IMMEDIATE DISCHARGE--JUST CAUSE FOR IMMEDIATE DISCHARGE SHALL INCLUDE: BUT
NOT BE LIMITED TO:
1-MISTREATMENT OF RESIDENTS/PATIENTS.
2-CARELESSNESS RESULTING IN INJURY TO RESIDENTS/PATIENTS OR OTHER
EMPLOYEES.
3-PROVEN THEFT
4-PROVEN DISHONESTY
5-GROSS INSUBORDINATION
6-GIVING OUT CONFIDENTIAL INFORMATION (MEDICAL INFORMATION OR RECORDS,
PERSONAL,PATIENT,RESIDENT OR EMPLOYEE RECORDS OR INFORMATION;
PERSONNEL RECORDS INFORMATION; OR BUSINESS OR FINANCIAL RECORDS,
INFORMATION OR CORRESPONDENCE) TO ANY UNAUTHORIZED PERSON.
7- REFUSAL TO PERFORM A SUBSTANTIAL AMOUNT OF REQUIRED WORK.
8- INTOXICATION, DRINKING,POSSESSION,USE, OR SALE OF ALCOHOLIC
BEVERAGES,OR PRESCRIPTION DRUGS-OTHER THAN BY A PHYSICIAN
OR PHARMACIST OR DOSAGE TAKEN, NOT IN EXCESS OF THOSE PRESCRIBED
BY A PHYSICIAN.
9-FALSIFICATION OF PERSONNEL OR OTHERWISE RECORDS, FALSIFICATION OF
TESTIMONY OR ANY VIOLATION OF THE OATH AS A WITNESS OR PARTY
IN ANY ACTION OR PROCEEDING OR HEARING BROUGHT AGAINST THE COMPANY,
EITHER DIRECTLY OR INDIRECTLY.
10-KNOWINGLY OR INTENTIONALLY ALTERING OR PUNCHING THE TIME CARD
FOR ANOTHER EMPLOYEE.
11-ENGAGING IN DELIBERATE SABOTAGE OR ACTS OR MALICIOUS MISCHIEF.
12-SMOKING IN AREAS WHERE SMOKING IS EXPRESSLY FORBIDDEN AND WHERE SUCH
SMOKING CREATES A SERIOUS HAZARD TO HEALTH AND SAFETY.
13-FAILURE TO PUNCH YOUR OWN TIME CARD, OR KNOWINGLY PUNCHING ANOTHER
EMPLOYEE'S TIME CARD.
IF YOUR TIME CARD HAS BEEN PUNCHED IN ERROR BY SOMEONE ELSE, GO
IMMEDIATELY TO THE "CHARGE NURSE" FOR CORRECTION.

DISCHARGE FOLLOWING WARNING NOTICES: A VIOLATION OF ANY OF THE FOLLOWING ACTS
AFTER HAVING RECEIVED TWO WRITTEN WARNING NOTICES IN THE YEAR IMMEDIATELY
PRECEDING THE VIOLATION WILL RESULT IN IMMEDIATE DISCHARGE. CAUSE FOR
ISSUANCE OF A WRITTEN WARNING NOTICE SHALL INCLUDE, NOT BE LIMITED TO:
1- FAILURE TO WORK UNTIL TIME DESIGNATED AS QUITTING TIME.
2- FAILURE TO BEGIN WORK PROMPTLY AT START OF SHIFT AND END OF THE
LUNCH PERIOD AND COFFEE BREAKS.
3- ABSENCE FROM REGULAR WORK STATIONS DURING WORKING HOURS
WITHOUT AUTHORIZATION FROM SUPERVISOR.
4- HABITUAL ABSENCE OR TARDINESS WITHOUT REASONABLE CAUSE.
5- FAILURE TO NOTIFY COMPANY OF REASON FOR ABSENCE FROM WORK ONE HOUR
BEFORE STARTING TIME.
6- CAUSING OR CREATING ANY UNSAFE CONDITION THAT COULD CAUSE BODILY
INJURY OR HARM TO SELF, RESIDENT/PATIENTS OR FELLOW EMPLOYEES.
7- DISREGARD OF SAFETY RULES OR COMMON SAFETY PRACTICES.
8- UNAUTHORIZED SOLICITING OR COLLECTING CONTRIBUTIONS FOR ANY PURPOSE
WHATSOEVER DURING WORKING TIME.
9- ENGAGING IN ANY PERSONAL WORK ON PREMISES WITHOUT MANAGEMENT APPROVAL.
10- USING ANY COMPANY EQUIPMENT WITHOUT WRITTEN APPROVAL OF MANAGEMENT
FOR OTHER THAN COMPANY BUSINESS.
11- USING COMPANY PHONE OTHER THAN ON COMPANY BUSINESS UNLESS PERMISSION
IS GRANTED BY SUPERVISOR OR RECEIVING PERSONAL CALLS, EXCEPT IN CASE
OF EMERGENCY.
12- CREATING OR CONTRIBUTING TO UNSANITARY CONDITIONS.
13- FAILURE OR REFUSAL TO PUNCH OUT AT THE BEGINNING OF THE LUNCH PERIOD
AND IN AT THE END OF THE LUNCH PERIOD.
<A> PUNCHING IN EARLIER THAN FIVE MINUTES BEFORE
STARTING TIME OR:
<B> PUNCHING OUT LATER THAN FIVE MINUTES AFTER QUITTING TIME.
<C> CORRECTION OF ERRORS MADE ON THE TIME CARD MUST BE
INITIALED BY YOUR SUPERVISOR.
