  15. Registering the Modem Doctor gives you access to one of the most powerful 
  16. diagnostic tools you can have for your PC. Registration is simple.  
  17. Price for the Modem Doctor is $19.95 which includes US and Canada shipping
  18. charges. Payment can be made by Check or Money Order. We can handle COD's,
  19. however all shipping and handling charges for COD's must be paid by you 
  20. and arraigned prior to shipment. Maryland residents should add the sales 
  21. tax, if appropriate.

  23. International Orders

  25. International Orders  should submit a money  order payable in US  funds. We
  26. cannot accept checks  drawn on  foreign banks that  do not have  a US  bank
  27. affiliation.  International orders should  also include $2.00  for shipping
  28. and handling (except Canada where shipping is included).

  30. Remember...shipping is  paid for  in the  US and  Canada except for  COD's.
  31. International  Orders must  include $2.00.  All  payments in  US FUNDS,  no
  32. checks drawn on a  foreign bank that does not have a  US affiliation can be
  33. accepted.

  50. If you are a registered user of an earlier version of the Modem Doctor, you
  51. can update  to Version 5.0 for $5.00 + $2.00 for shipping.  All you need to
  52. do  is include your  Modem Doctor Serial  Number (on your  original disk or
  53. available when  you first start  the Modem  Doctor) with this  form. Please
  54. fill out  the  serial number  information so  we can  send  you the  latest
  55. version of the Modem Doctor.
