  24.     If at any time I should have an incurable injury,

  26. disease, or illness certified to be a terminal and irreversible
  27.  
  28. condition by two physicians who have personally examined me, 
  29.  
  30. one of whom shall be my attending physician, and the physicians 
  31.  
  32. have determined that my death will occur whether or not life-
  33.  
  34. sustaining procedures are utilized and where the application
  35.  
  36. of life-sustaining procedures would serve only to artificially
  37.  
  38. prolong the dying process, I direct that such procedures be
  39.  
  40. withheld or withdrawn, and that I be permitted to die
  41.  
  42. naturally with only the administration of medication or the
  43.  
  44. performance of any medical procedure deemed necessary to
  45.  
  46. provide me with comfort care.
  47.  
  48.     In the absence of my ability to give directions
  49.  
  50. regarding the use of such life-sustaining procedures, it is
  51.  
  52. my intention that this declaration shall be honored by my
  53.  
  54. family and physicians as the final expression of my legal right
  55.  
  56. to refuse medical or surgical treatment and accept the
  57.  
  58. consequences from such refusal.
