  5.      Please fill out all applicable sections, then mail this form and
  6. required proofs of purchase to:

  9.         Minneapolis, MN 55440-1541
  10.      Please complete one form for each qualifIed location (see Notes).
  11. If you need assistance, call 1-800-677-2581 from 9:00 a.m. to 5:30 p.m.,
  12. Eastern time, Monday through Friday.
  13.      All earlier OS/2(R) Standard Edition versions must have been acquired
  14. on or before March 31, 1992. Your upgrade order must be received on or
  15. after March 31, 1992, but no later than August 24, 1992.
  16.      All earlier OS/2 Extended Edition and LAN Server versions must have
  17. been acquired on or before April 24, 1992. Your upgrade order must be
  18. received on or after March 31, 1992, but no later than August 24, 1992.

  20. Section 1: For IBM OS/2 Version 2.0 Retail Upgrade Package
  21.      Fill out if you are upgrading from IBM OS/2 Standard and/or Extended
  22. Edition Version 1.0, 1.1, 1.2 or 1.3.

  113.      For licenses being upgraded, you'll receive one upgrade package. In
  114. addition, you'll receive Proof of Additional License certificates for
  115. upgrading all other earlier version licenses.
  116.      Only one order form can be used per qualifying location (a single
  117. building designated by a single mailing address in the United States or
  118. Puerto Rico). Photocopies of this form can be used to place orders for
  119. additional locations.
  120.      Lost or misdirected mail is not the responsibility of IBM or the
  121. IBM Redemption Center.
  122.      IBM reserves the right to audit any customer submission that is not
  123. accompanied by a purchase invoice, proof of license or proof of
  124. additional license.
