  2. HIV-Related Service Categories
  3.  
  4. 1.   PRIMARY MEDICAL CARE: Provision of routine, non-
  5.      emergency, non-inpatient, non-specialized medical care.
  6.  
  7. 2.   DENTAL CARE: Diagnostic and therapeutic services 
  8.      rendered by dentists, dental hygienists, and similar 
  9.      professional practitioners.
  10.  
  11. 3.   MENTAL HEALTH THERAPY/COUNSELING: Psychological and 
  12.      psychiatric treatment and counseling services, 
  13.      including individual and group counseling, provided by 
  14.      a mental health professional licensed or authorized 
  15.      within the State, including psychiatrists, 
  16.      psychologists, clinical nurse specialists, social 
  17.      workers, and counselors.

  19. 4.   CASE MANAGEMENT: Client-centered service that links 
  20.      clients with health care and psychosocial services in a 
  21.      manner that ensures timely, coordinated access to 
  22.      medically appropriate levels of care and support 
  23.      services, and continuity of care. Key activities 
  24.      include: assessment of client's needs and personal 
  25.      support system; development of a comprehensive, 
  26.      individualized service plan; coordination of services 
  27.      required to implement the plan; client monitoring to 
  28.      assess the efficacy of the plan; and periodic 
  29.      reevaluation and adaptation of the plan as necessary 
  30.      over the life of the client.

  32. 5.   HOME HEALTH CARE: Therapeutic, nursing, supportive 
  33.      and/or compensatory health services provided by a 
  34.      licensed/certified home health agency in a 
  35.      home/residential setting in accordance with a written, 
  36.      individualized plan of care established by a case 
  37.      management team that includes appropriate health care 
  38.      professionals. Component services are defined 
  39.      separately below:
  40.  
  41.      a.  Para-Professional Care: Homemaker, home health 
  42.          aide, and personal/attendant care. 
  43.      b.  Professional Care: Routine and skilled nursing, 
  44.          rehabilitation and mental health.
  45.      c.  Specialized Care: Intravenous and aerosolized 
  46.          medication treatments, diagnostic testing, 
  47.          parenteral feedings and other high tech services.
  48.      d.  Durable medical equipment: Prosthetics, devices and 
  49.          equipment used by clients in a home/residential 
  50.          setting, e.g., wheelchairs, inhalation therapy 
  51.          equipment or hospital beds.

  53. 6.   REHABILITATION CARE: Services provided by a licensed or 
  54.      authorized professional in accordance with an 
  55.      individualized plan of care which is intended to 
  56.      improve or maintain a client's quality of life and 
  57.      optimal capacity for self-care. The definition includes 
  58.      physical therapy, speech pathology, and low-vision 
  59.      training services.

  61. 7.   SUBSTANCE ABUSE TREATMENT/COUNSELING: Treatment and/or 
  62.      counseling to address substance abuse (including 
  63.      alcohol) problems.
  64.  
  65. 8.   HOSPICE CARE: Nursing care, counseling, physician 
  66.      services, and palliative therapeutics provided by a 
  67.      hospice program to patients in the terminal stages of 
  68.      illness in their home setting.
  69.  
  70. 9.   SUPPORT SERVICES:
  71.      a.  Adoption/Foster Care Assistance: Assistance in 
  72.          placing children under the age of 20 because their 
  73.          parents are unable to care for them due to HIV-
  74.          related illness or death, in temporary (foster 
  75.          care) or permanent (adoption) homes.
  76.      b.  Buddy/Companion Services: Activities provided by 
  77.          volunteers/peers to assist the client in performing 
  78.          household or personal tasks, and providing mental 
  79.          and social support to combat the negative effects 
  80.          of loneliness and isolation.
  81.      c.  Day and respite care: Residential or home-based 
  82.          non-medical assistance designed to relieve the 
  83.          primary caregiver responsible for providing day-to-
  84.          day care of client or client's child.
  85.      d.  Direct Emergency Assistance: Provision of short-
  86.          term supports for food, housing, rent, utilities, 
  87.          medication or other resources. 
  88.      e.  Food Bank/Home Delivered Meals: Provision of actual 
  89.          food or meals, not finances to purchase food or 
  90.          meals.
  91.      f.  Housing and Housing-Related Services: This 
  92.          includes: assistance in locating and obtaining 
  93.          suitable, on-going or transitional shelter 
  94.          (including costs associated with finding a 
  95.          residence and/or subsidized rent); and residential 
  96.          housing services, which are the provision of 
  97.          housing assistance in a group home setting.
  98.      g.  Client Advocacy: Assistance provided to individuals 
  99.          with respect to wills, funeral arrangements, 
  100.          matters related to protection of civil rights, and 
  101.          other relevant needs experienced by clients.
  102.      h.  Transportation: Conveyance services provided to a 
  103.          client in order to access health care or 
  104.          psychosocial support services. May be provided 
  105.          routinely or on an emergency basis.
  106.      i.  Other: Support services not listed above, including 
  107.          (but not limited to) non-mental health counseling, 
  108.          translation and interpretation, etc. 
  109.      j.  In-Patient Personnel Costs: Within the limitations 
  110.          of the legislation (see Sec. 2604, (c)(l) through 
  111.          (3)) up to ten percent of the total award for such 
  112.          costs upon determination by the HIV Health Services 
  113.          Planning Council that a shortage of inpatient 
  114.          personnel exists which has resulted in 
  115.          inappropriate utilization of inpatient services.
