  6.     You are hereby required to pay the rent on the premises
  7. herein described, of which you now hold possession, pursuant
  8. to a written lease, amounting to $          , being the rent
  9. now due to me by you for the period from        , to       ,
  10. or you are hereby required to deliver up possession of the
  11. premises, within THREE DAYS after service on you of this
  12. notice, to the undersigned or the undersigned will institute
  13. legal proceedings against you, to declare a forfeiture of
  14. the lease under which you occupy said premises and to
  15. recover possession thereof, with treble rents and damages.
