- Log Sheet or Spreadsheet
- A log sheet is an alternative or adjunct to software. There are a lot of
- different log sheets out there, but the best, in my opinion, is one
- developed by Jean-Fran+AOc-ois Yale, M.D., Associate Professor of Medicine,
- Crabtree Nutrition Laboratories, McGill Nutrition and Food Science Centre,
- Royal Victoria Hospital, and Director, Metabolic Day Centre. I met with him
- in Montreal in August 2002, and at that time he authorized me to make his
- log sheet available on my Web site. Dr. Yale's log sheet lets you enter
- blood glucose readings, insulin dosages, carbohydrate grams, and exercise
- (which you can put on a scale of 1 to 5) for any hour of the day. Before
- printing the log sheets remember to change the page layout to landscape and
- when you are finished to change it back to the usual portrait layout. Two
- pages of the log sheet cover the seven days of the week. It is an Adobe
- Acrobat PDF file, which requires the free Acrobat Reader, available at
- http://www.adobe.com/products/acrobat/readstep.html, if you don't have it

- The Online Blood Sugar Log lets you keep track of all your blood sugar test

- CWD CoPilot allows patients to upload meter and insulin dosing data to an
- online database. CWD CoPilot can generate graphs and charts from the data to
- help patients identify patterns. The data can also be shared with a
- patient's health care team. (There's a separate HCP registration area.)
- While CWD CoPilot is sponsored by TheraSense, it supports the use of any

- DailyRating.com offers a question for you to answer on +ACI-What was your Blood

- bar charts from your readings. The neatest feature of this site is that you

- free on-line software that allows you to automatically upload data from any
- one of several meters to track your blood glucose levels. Just connect your
- meter to one of your computer's serial port with the meter's cable, click a
- couple of times, and your data is displayed in easy-to-read graphs that can
- be customized to show your blood glucose levels according to day, week,
- month, or by events such as exercise or meals. The program also graphs
- insulin.

- but with a graph, giving a picture of what your BG has done in a day. The
- other, called Glucose, Time Carbohydrate Diet, allows users, in consultation

- Health is freeware software application for people with diabetes and/or
- hypertension (high blood pressure). It records and graphs your blood
- glucose, blood pressure, weight, and A1c values. The BG values can be the

- My Diabetes Assistant
- Raul Serrano says that he developed his program, My Diabetes Assistant, to
- assist people with diabetes in tracking some of their everyday activities.
- MDA is a reporting system in which you input data of as many daily
- activities you wish that relate directly to your glucose control. From
- simple glucose reading to meals, weight, blood pressure, etc., you decide
- which you need to monitor. By reviewing/analyzing the reports you can
- quickly decide how you're doing in the specific areas, and make needed
- changes to your glucose management if needed. The VB6 runtime files must
- exist on your system in order for MDA to run. You can download them from a
- link on his site. MDA is freeware. A commercial version is also available.

- NutriSoft sells three similar programs that sound good, but none of them can
- be recommended for people with diabetes who want to use software to track
- how their medications and/or insulin, exercise, diet, weight, and state of
- health affect their blood glucose levels. None of these programs have
- anything but the most basic blood glucose tracking, and all of them require
- you to enter your blood glucose levels manually. NutriGenie makes versions
- of its programs available on-line at many sites, but all of those for
- diabetes management are severely crippled (they have limited databases and
- permit nothing to be saved). Perhaps an even more serious criticism of
- NutriGenie's products is that the company is essentially unreachable. It
- does not provide a street address, only a post office box number. It does
- not provide a telephone number, whether toll-free or local. It has three
- e-mail addresses (nutrigenie+AEA-yahoo.com, NutriSoft+AEA-aol.com, and
- 71563.124+AEA-compuserve.com, but e-mail directed to any of these addresses goes
- unanswered.

- MedMonitor version 1.01 from DataMass Software Inc. of Washington, Missouri,
- phone (314) 390-2584, is apparently no longer available. It offered Windows
- 95 software to monitor your medicine. According to e-mail from Tim Albrecht,
- the vice president and founder of DataMass, +ACI-The MedMonitor V1.01 will help
- you keep an inventory of your medications, help you keep track of what
- medications you have taken and what you have not, helps you keep track of
- your prescriptions remaining for each medication and prescription numbers,
- glucose test results and produces a properly formatted test report for your
- doctor to be able to adjust your medications+ADs- it also helps you keep track
- of changes your doctor makes in your medications or related devices, it
- provides necessary information for you to be able to tell if your insulin is
- fresh or not, and it even has a calendar built in to keep track of your
- appointments and milestones. The normal cost of MedMonitor V1.01 (Diabetes)
- +AFs-is+AF0- only +ACQ-45.00 after a +ACQ-20.00 mail-in rebate...the price to U.S. Veterans

- Mellitus Manager Version 4.5 by MetaMedix Inc. of Escondido, California, was
- discontinued in September 2000, following the iMetrikus Inc. acquisition of
- MetaMedix.

- According to Children with Diabetes's +ACI-Computerizing Your Meter+ACI- page (see
- above) Drug ID International Inc. sold THE QUANTITATOR software which,
- according to Drug ID, converts the daily insulin regimen into hourly amounts
- of insulin released to the body. However, this site is no longer reachable.
