Trigger Thumb Overuse Injury

QUESTION: I have been having trouble with my right thumb. I went to the
doctor and he says I have "trigger thumb". What exactly is that? He
prescribed anti-inflammatory medication and said it might eventually mean
surgery. Can you explain this diagnosis, please? Thank you, your column is

ANSWER: I am sure your doctor knows his medicine, but question his knowledge
of fire arms. Ever try to pull a trigger with your thumb? I think he is
applying another term "trigger finger" to a similar problem in your thumb. To
flex your finger, the action that describes pulling a trigger, the flexor
tendons are activated by muscles. These tendons pass through a sheath, a
tunnel like structure that helps to lubricate and guide the tendons. When
there is excessive use, the sheaths may become inflamed, causing pain and
restricting motion. Treatment can consist of anti-inflammatory medication,
injecting corticosteroids into the area, or eventually surgery. Surgery is
necessary when there is scarring of the sheaths, the result of chronic
inflammation, and opens the sheath to free the tendons and allow them to move
freely. I must assume you have a flexor sheath problem in your thumb, and
your physician is associating this with the more commonly described "trigger
finger". Since the mechanisms and treatments are both the same, he is not
that far off the target (hah!).
