A. The applicant is of the full age of 18 years or older or is the legal guardian of the applicant.

B. The applicant consents to the use of their name, and likeness in connection with SUPERMODEL.COM's "Club MSA", for use on any media or for related promotional purposes, throughout the world in perpetuity. The applicant grants SUPERMODEL.COM's "Club MSA" the right to edit, format or otherwise alter any such material as necessary.

C. The applicant releases SUPERMODEL.COM and Model Search America, Inc., their officers, directors, and employees, from and against any and all claims which the applicant has or may have for invasion of privacy, defamation or any other cause of action arising out of the use of the member's name and likeness and or any and all related materials in connection with SUPERMODEL.COM's "Club MSA".

D. That SUPERMODEL.COM and Model Search America, Inc. have made no promise, representation or warranty that the undersigned will secure any form of employment or enter into any other contract as a result of their participation in SUPERMODEL.COM's "Club MSA".

E. That this membership form may be rejected at the sole discretion of SUPERMODEL.COM and Model Search America, Inc. in which case the applicant's fee will be returned in full. However, SUPERMODEL.COM's "Club MSA" is not responsible for the return of any photographs or additional materials of any kind submitted by applicant.

F. Applicant acknowledges the potential risks involved with participating within an online environmet, including the receipt of unrequested contacts and/or solicitations. SUPERMODEL.COM and Model Search America are not responsible for the content of any electronic mail sent by or recieved from applicants while using this service.

G. That this is the entire agreement of the parties.

SUPERMODEL.COM's "Club MSA" is owned and opperated by SUPERMODEL.COM and Model Search America, Inc. SUPERMODEL.COM is a division of Patxi Entertainment Network, Inc.
