  30. order in US dollars and drawn on a US bank.  (Residents of Canada can pay

  33. Residents of New York State should add 8% sales tax, and customers outside
  34. North America should add $5US per copy shipped.

  50. and are expected to make their own copies for on-site distribution.  A
