  4.         The Hayes User Group Program is a special purchase program which
  5. allows qualified members of User Groups to make purchases of Hayes
  6. products at discount prices.  The User Group Program reinforces Hayes
  7. presence in the User Group community and continues a close relationship
  8. between User Groups and Hayes.
  9.  
  10.         In addition to special purchase program, Hayes can also provide
  11. speakers to inform your group about Hayes products and other opportunities.

  14. Procedure for ordering products through the Hayes User Group Program :
  15.  
  16. 1) Select the items that you would like to purchase from the User
  17.    Group Price List.
  18.  
  19. 2) Fill out the order form, be sure to use the correct part number!
  20.  
  21. 3) Return the order form to Hayes either by mail or fax, the address
  22.    and phone number is on the order form.
  23.  
  24. 4) Upon approval, your order will be shipped directly to you at
  25.    the shipping address you provided on the order form.
  26.  
  27.  
  28.   Hayes reserves the right to require additional information,
  29.   documentation or other items as deemed necessary to verify that the
  30.   purchaser is indeed a member of a qualified User Group.
