  5. long distance customers who can provide a recent long distance bill showing 
  6. a minumum $500.00 monthly long distance paid usage. Note that rates marked 
  7. "Dedicated" are for customers with a T-1 or similar 24 line switch 
  8. installation. All rates must be pre - approved through MagNet Sales Office.
